Photo/Publicity Release Opt-Out Form
I have questions about the Photo/Publicity Release Form. In it is states that the directory information includes, but is not limited to the following data:
Name of student
Address
Date and place of birth
Birth certificate serial number
Telephone listing
Dates of attendance
Participation in officially recognized activities
Height and weight
Awards and Honors received

This is a consent to all or nothing release form. I signed this form because I did not feel some of this information needed to be released to the public, especially since my children are minors and it it my responsibility to protect them.

Now I have received a letter stating that my child will not have his photo or name in the yearbook or the paper when he receives awards. Those are things I feel are okay, but the thought of not knowing when his address, phone number, date and place of birth, birth certificate serial number are released are not okay with me, unless I am informed beforehand and know why such people or organizations would need that information.

Is there any way to pick and choose what is allowed?

Thank you,
Heather Walton

RE:Photo/Publicity Release Opt-Out Form
Currently it is a blanket approval or refusal; however we will review your request to see if that option is manageable. If so, changes will start in 2013-14. In the meantime, you may send a letter to the school providing permission for the yearbook photo and awards.
